Many organizations have a certified management system but still struggle with repeat problems. The missing ingredient is often culture.

  • People feel safe to report problems and near misses
  • Teams use data to make decisions
  • Improvement is part of everyone’s job, not a separate project

How to build it

  1. Lead by example — leaders must act on quality issues visibly.
  2. Engage people — involve front-line staff in problem solving.
  3. Recognize good practice — celebrate improvements, not just outcomes.
  4. Measure what matters — use a few meaningful indicators.
  5. Learn from failure — focus on systems, not blame.
